I have a list of 500+ accreditation standards for a school and a form that selects sub-sets based on type of school and method (interview, training observation, etc.). I need to be able to enter data fields (comments, score, etc.) for each one and then save the form to a data connection in SharePoint so that the data can be analysed. I have the list items showing properly using button selections and can't figure out how to have a unique field for comments, for example, for the criteria showing on the list. The field repeats the input for all list items showing rather than allowing unique inputs.
