Choose which fields to be completed

I want to create a repeating table.  Each row would ideally allow the person completing the form to choose from a list which field they want to complete.

In this example, the table is a set of human health information.  I would like for the user to be able to choose if they want to provide Height, Weight, # of pushups completed, ect.

The table would then repeat and allow them to choose another piece of information from the same list.

any ideas on how to accomplish something like this?

here is a partial idea, but it's not complete:

    1.  create a drop-down list control.  populate the choices using a list in sharepoint. The sharepoint list would use the title column to store the name of an activity, such as height, weight, pushups, ect.  these titles would correspond to optional fields in the main datasource.

    2.  create a rule that would somehow add the field with the corresponding name (using a data filter) to the form after it has been chosen from the list.

    3. create a rule that would hide the selection box from Step 1 above.

    4. repeat

I have no idea how to accomplish step 2.  everything else seems like things I have done before. again, any ideas would be super helpful.
