Code for hardware request form template in InfoPath - Used with SharePoint

Hi All,

I am working on hardware requests via SharePoint library.

I found some instructions on MSDN, but the code they had for the InfoPath hardware request template seems to be no longer available, and it would save me a huge amount of time if someone can assist or even better provide the same code or even a similar code, which will allow me to concentrate in getting workflows and everything else working?
