Best way to collect the same 5 pieces of data multiple times in a filler form

I am building a filler form for doing a site inspection. The form is long, but there are 5 (optional) common fields that are used throughout. If the user enters that an item needs Immediate Repair, 5 additional pieces of information have to be completed. I am including the fields in the data source. What's the best way to do this? Switching to another view doesn't seem to work. Do I need to build a custom Windows form to enter the additional information? Then I would need to pass the XPATH of the group to the code. Thanks in advance.

Add validation rules to the fields for the 5 additional pieces that makes them only required if the other field = Immediate Repair. You can also put the fields inside a section that you hide unless the other field = Immediate Repair.

    Hi Hilary,

I don't think I want to show those pieces of information in the form.  There are too many of these 'Status' dropdowns and it would really mess up the form to add all those additional fields.  I was thinking of putting that information in another view. I would maybe color the background of the dropdown if the data existed.

    Thanks,

    Susie
