Using an InfoPath web part to sign a Policy.

I have added our company's IT Usage and Cell-Phone Policies to SharePoint. It is all well that they can be read on-line, however we need the forms to be signed by all staff highlighting that they have read and agree to the policy. Initially by creating a simple form, and adding it as a web part to the bottom of the Policy this was instantly achievable by setting the form to submit and save as the name field. However, it has been decided that each form from every policy must populate one SharePoint form library. This is a problem when only one form per name may be submitted to the Library. A URL Query filter has been added to each policy to populate a "policy" column in the library using a hidden field in the form. We have also added a second view, so that when the form is submitted, a new form opens for that user stating that they have already submitted a form. A problem with this is that once one form has been submitted, this second view will show up on all policies. We need a way around these issues, so that the same form can be submitted from different web parts and populate the same library. Any help would be hugely appreciated.
