Users enter the name and zip of the student and it launches a Infopath form and the code behind this is as following:

however I would like to launch the form which is in the following
location after the infopath form is published in the form library and
the location is

when the form is opened using the publishing Wizard the address is as following:

How do I write the above string href in the following location?
