    Formula....maybe? Need Some help!

So I have created a browser based infopath 07 form  at a large healthcare organization.
    - In the first field the user enters their name from a drop down box. (This  box has a data-connection to a list on sharepoint. The list on sharepoint has different columns ,ie their name, email, clinic name, and location.)
    - The user than fills in multilple other sections with information and those work just as they should. At the end of the form the user clicks the submit button which sends the form to a document library and emails the form to their supervisor, w/ attachment.
    - The supervisor reviews than clicks submit button in their section which sends an email to the chief.
    - The chief will review the email from the supervisor and will click submit in their section.
    - The emailed form than goes to the schedulers to complete.

While the form is going throught the process it is sending information to the document library on sharepoint updating the forms progress through the steps.

    My ISSUE-
------  When the chief submits his approval I would like the email that is sent to the schedulers to have the subject that corresponds with that  users clinic name and the location. Is there a formula (or way) that can put the clinic name and location in the subject line of the email. So basically the subject would be Clinic A and Location A need to closed.  That way when the scheduler looks at the email subject they know who and where this needs to be sent.

    HELP PLEASE!!!

When you set up the E-mail data connection, you can click the Fx buttons to set up a formula for the subject line.  Then just use the Insert Field of Group... button to select the fields you want to use, and the concat() function to string them together:

    concat(ClinicName, ' ', LocationName, ' needs to be closed.')

I have done that and it returns the clinic name of the person at the top of the list.

So i choose Person D on my drop down list, when i put in concat(ClinicName,ClinicLocation) in the subject it displays person As clinic name and location..... when the email is sent.

I have found it easier to create a hidden field called Subject, populate the value with rules. This will also allow you to make the subject more robust and dynamic. This string should be available when your ready to email. Although I do believe Jimmy's response should work I have had a few times when I had to throw logic out the window to figure out a way to solve an issue
    Kind regards,

I'm having trouble understanding what you're describing in your second post:

I have done that and it returns the clinic name of the person at the top of the list.

What list is this that you're referring to?

I don't understand what this means:

in the subject it displays person As clinic name and location..... when the email is sent.

    Re: Formula....maybe? Need Some help!

when I put in concat(clinicname, cliniclocation) in the subject title (using fx) it only gives me the the first clinic name and first clinic location that is on the list.

So what i did was go to the data connection > modify > fx > insert function> concat() > clinicname > cliniclocation > ok > than publish.

So if "doug" makes the request and selects his name in the drop down box it will submit the email but the subject will show the clinic name and location of "allison" in the subject title of the email (because she is the first one in the list)

Here is what it looks like

Could you attach a screenshot of your form's main data source (from the taskpane on the right)?

When you select the fields for your formula, you need to click the Filter button, and set up a condition to pick the ClinicName/ClinicLocation where provider equals the providerName in your main data source.  That should do the trick.
