Is it possible to put the DS picture or name in the form body? I haven't found any answer for this anywhere.

Do you want to place the certificate picture and its details? If so, just take a picture of the certificate. Add a picture control and set that picture as the default picture.
Signed by name will be displaye in the form once user sign the form. Is your question is, do you want to show the DS picture or name in the email body?
