And when you say "when it is submitted" you mean in the sent email? Sorry, just trying to clarify the problem so I know what I'm looking for.

I should add - have you considered having a single field for the description and then using a secondary data source (like an XML or SharePoint list) for the values? Filtering could be used to only display the values based on the selection in the first dropdown (I can help you with that if you need it) .... I'm also a bit confused by the design with the need to insert a row and click a checkbox to see the dropdowns - was there a requirement for that? It seems odd to me - that the user first clicks to insert a row, then clicks a checkbox in the row before any controls show up?

yes, submitted to the email. I have not considered that to be honest and I would appreciate your help on that as I would not know where to start on that. no really a requirement. I made the design like that in case they wanted additional rows. Do you think I should just let them be able to add additional rows and the textbox just appear?

Yeah, I would just let them add rows and have the controls show up. I'll get you an updated form later today that uses a single description field and an XML file for the values, so you can see how that works. Then you don't have quite as many conditional formatting rules to maintain.

Save the file linked in the header of this post locally (right click the link and select "save target as" or whatever the menu option is for your browser). Once you've saved the file, right click and select design to open in design mode. I just stuck a new table in with the component and description dropdowns and some screenshots & info on how it was implemented.

Hi Hilary, I went ahead and tested what you shared with me. It seems to be giving the same output. see attached image.

Well - that screenshot doesn't seem to show the new table / controls I put on the form? Did you preview and try those? Where you don't need a bunch of description fields with conditional formatting?
