I have twenty follow up date/times and I need to create a rule that if date/time is not blank and form is submitted it is disabled
And the cycle repeats for the next time they open the form in edit mode to enter another follow up in date/time field

You need to set a formatting rule on the field, the condition would be if the date field is not blank->disable the control. I am attaching a sample template DateFieldFormatting.xsn.
