You can add a link in your form that opens a Google Map. Use a function like this:
concat("http://maps.google.com/maps?q=", my:Adress, ", ", my:Address2, ", ", my:City, ", ", my:State, " ", my:Zip, " ")
If Google has an REST API that returns an image, you can also include that in the form.
