forms stopped opening - system.security.permissions.urlidentitypermissions, mscorlib

All of a sudden our qdabra qrules forms started error'ing.  I looked at web.config and restored to a known good one.  I've also re-admin published one of the forms but continue to get this error.

    11/14/2012 11:29:20.95 w3wp.exe (0x1DE4) 0x2A1C SharePoint Foundation Runtime 6615 Critical Request for the permission of type 'System.Security.Permissions.UrlIdentityPermission, mscorlib, Version=2.0.0.0, Culture=neutral, PublicKeyToken=b77a5c561934e089' failed. 112082dd-4348-468c-88dd-a0ec8cf7cbb4

Opening in IPFS? You say admin published and have a line from your log, so I assume this is not Office 365? Did you recently make updates to your SharePoint server?

    server SP 2010 not 365

    InfoPath 2010

form published to form library and set to open in browser mode

The form works (opens) fine in the filer (IPFS)

only change today: admin published another form earlier today.  That one is working fine.

    You'd said:

I thought you meant all of them. Is there only one that this is happening with?

Spoke to soon.  it's only this one form.  It does have qrules injected.  I just tested  another that has qrules injected and it is working fine.

weird is that it was working fine this morning.. i can see several entries in the library today.  Now this error.  I've searched online but don't really see anything for system.security.permissions.urlidentitypermissions.

I created a new form library and re-admin published the form as a different name, added that form to the new library, but get the same error.

I can try publishing your form on one of our servers if you like, see if we also get an error - feel free to email it to me or attach it to a reply.
