For emailing I usually resort to using expression boxes, whose formatting in my opinion looks cleaner in email readers. So I'll have two sections: a section for filling out the form (hidden in emails) and a section for emailing (hidden while filling out the form). Same data, just different controls and different layout.
I'm not sure why your Date Picker would have enormous width. It would take some time troubleshooting, looking at the form, etc. I guess you could try placing the controls inside a table, and set that table's width such that the form will never exceed that width.
