I remember back in InfoPath 2003, I'd place a drop down and a text box bound to the same field. One option in the drop down would be 'Other ...' and then I'd conditionally format the drop down to hide and the text box to show if the value of the drop down was 'Other ...'. That way if the user selected Other, they'd have a text box to type into.
