Attached template. I have tried several different options to create a "Save" button and a "New Request" button with the following requirements.
Save - saves current values to SharePoint list. Pulls up new record, with all fields blank.
New Request -
checks if current record has been saved (unless not all required fields filled), if record has been saved, clears all fields and starts with new record;
if record has not been saved, user is prompted to proceed without saving current record, deleting current field contents, or save current record with values, and clearing all fields
Current issues - logic for dirty flag if this is needed, clearing all fields and not displaying first record or any other record
