Multiple-Selection List Boxes are not currently supported. Replace the multiple-selection list box control in your form with a dropdown control in a repeating section. Click Download to see the sample.

Combo Box Solution

Combo boxes are not currently supported. Create a dropdown and add Other option to it, use the Other option to conditionally hide the dropdown control and show a text box control that allows free text entry. Add a button to the right of this control that toggles back to the dropdown. Click Download to see the sample.

Scenario:

You’ve uploaded the FormsViewer app in the App Catalog and approved the app. When elevating permissions in FormsViewer, you might encounter a message saying that "Your tenant adminstrator has to approve this app" even though the app is already approved.
