Cascading Dropdown list in infopath 2003

Hello, I was wondering if anyone can help me with my problem? I have tried all the tutorials on line I could find. I am working on a project where I need cascading drop down list (about 5 drop downs each depending on the prior one), but I can't seem to get it to work. I'm retrieving my data from MS access 2003. I don't know if my database is the problem?( is their a specific way the database has to be set up?) My drop down list would include: 1. Nursing Diagnosis 2. Risk Factors 3. Plan of Outcome 4.Intervention' 5. Rational for intervention Thanks

For me, after reading and re-reading the question and after looking into the attached from template,  it is not clear what are the data (and their structure) to be put in drop-down list boxes and how they are related to data retrieved from MS Access 2003

my data will contain something like the file I uploaded in this post. I don't have the real data because it's at the place where I'm working on it. I want the cascading list to be if I choose Diagnosis (A), then the risk Factor (A) will be in the next drop down instead of risk factor A-E.(btw there could be more than one risk factor for diagnosis A. ( ex. risk factor A.1, A.2 A.3). I want to know how to create a cascade and create the database the right way for the data I have. Every time I try follow the steps of tutorial they never work. The data usually populates in one list, but not the others.
